저는 한림대학교 강남성심병원 산부인과에 근무하는 교수 이근영 입니다.

 지금으로부터 11년 전 미국 산부인과학회에 참여하여 아이디어를 얻은 후, 양막이 돌출된 자궁경관무력증 산모에게 처음 시도한 응급 자궁경부 봉합술 환자가 생각납니다. 당시 외래를 방문한 산모는 임신 24주째로 자궁 경관이 개대되고 태아를 싸고 있는 양막이 자궁경관 밖으로 돌출되어 아무런 조치가 없으면 곧바로 태아를 분만할 상황이었습니다. 여러번의 조산 후 어렵게 가진 아기였기 때문에 할 수 있는 방법은 모두 해보기로 산모와 협의 하였습니다.

 산모 복부를 통하여 양수를 뽑아내는 양수 감압술 후 응급 자궁경관 봉축술을 시행하여 돌출된 양막을 다시 경관 안으로 밀어 넣고 개대된 자궁경관을 묶어서 임신 38주까지 지속시키고 건강한 2.8kg의 태아를 분만하였습니다. 이후 자궁경관무력증에 관심을 갖고 양막이 돌출된 자궁경관무력증 산모에게 응급 자궁경관봉축술을 시행하기 시작하여 이제는 200건 이상의 시술을 하였고, 약 57%의 태아를 생존시키고 있습니다.

 자궁경관무력증 산모, 과거 병력으로 이미 자궁경관무력증으로 진단받은 산모, 임신 12~14주 사이에 예방적 목적으로 자궁경관봉축술을 받는 산모 및 과거 질식자궁경관봉축술(맥도날드 수술 또는 쉬로드카 수술)을 시행하여 실패한 경험이 있거나 혹은 자궁경관이 너무 짧아서 질식자궁경관봉축술이 어려운 산모에게도 복식자궁경관봉축술을 시도하여 현재 약 250예를 시행하였고 대부분 좋은 결과를 얻었습니다. 최근에는 자궁경부암 발견 후 질식자궁경부 광범위 적출술과 복강경 임파절제술 후 임신된 산모에게 복식자궁경부 봉합술을 시행하여 생존한 쌍태아를 분만한 세계최최의 성공사례도 있습니다.

 자궁경관무력증과 관련하여 전국에서 매우 많은 산모들께 문의가 들어오고 직접 방문 하시는 분도 계시고 여러 산부인과 선생님들께서도 자궁경관무력증 산모를 보내주십니다. 약간 부담스럽기는 하지만 자궁경관무력증에 대하여 좀 더 자세히 알려드리고 도움이 필요하신 산모들께 보탬이 되고자 홈페이지를 열었습니다.

I write in public recognition of your innovative work in the field of Maternal
Fetal Medicine, especially as it relates to your ‘State of the Art’ approaches
for research and clinical management of cervical dysfunction leading to
pregnancy loss and preterm delivery. I base my opinions on more than
20 years of extramurally sponsored research and my writing in the area.

The clinical unit under your direction should be a source of pride for Hallym
University. The activities of your unit for the treatment of these disorders
include emergency or urgent rescue cervical cerclage, transabdominal
or transvaginal cervico-isthmic cerclage, and the development of new
surgical methods to improve outcome.
The conclusion that this is high caliper work is supported by peer-reviewed research publications in high quality journals in the United States, Japan and Korea.
I am especially impressed your many research abstracts presented
annually over the last few years at the internationally prestigious Society
for Maternal Fetal Medicine.

Lastly, I write because I am happy to call you my colleague in a concerted
effort to improve the health care of women worldwide. Our joint research
project on rescue cerclage resulted in the development of new biomarkers
predictive of the operation’s success. I am especially excited that we will
be initiating new translational research based on our results, bringing new
hope to couples who experience pregnancy loss.
